The Vial of Life is an international program designed to alert emergency personnel to the location of vital medical information in the home. What if an emergency occured and you couldn’t speak for yourself or a loved one? What if the stress or injury makes it impossible to remember what medications you’re on, who your emergency contacts are, or important facts about your medical hsitory?
Anyone who takes prescription medication or has a chronic illness should take advantage of this FREE program.
EMT’s and Fire personnel are trained to look for the Vial of Life symbols in homes, automobiles, or wallets to access your vital, and potentially life-saving, medical information in emergency situations.
The Vial of Life kit consists of:
- A plastic “pill” container;
- A Vital Statistics Information Form that contains your:
- Identification (Social Security Number, Birth Date, Marital Status);
- Emergency Contacts
- Advance Directive/DNR Orders
- Medications;
- Allergies;
- Medical Coverage;
- A door or window EMT alert sticker;
- A Vial of Life refrigerator magnet.
Follow these Simple Instructions:
- Fill out the medical information Form.
- Fold it (along with a copy of your Advance Directive, if you have one) and put it inside the plastic capsule.
- Place the capsule INSIDE of your refrigerator: either inside the door compartment or on the top shelf.
- Attach the Vial of Life magnet to the front of the refrigerator door.
- Tape the Vial of Life EMT alert card in a visible place so that Emergency Personnel could easily see it when they arrive – in the front window or on the front door.
REMEMBER TO KEEP YOUR INFORMATION UP TO DATE. If you travel or spend a lot of time in your car, attach a cross to the glove compartment and put the Vial inside. EMT personnel can not search your glove compartment unless there is a red cross on it. Please let us know if you require additional capsules or magnets.
